STEP 1. Gather the following documents according to the purpose of your trip:
Tourist & business requirements
- Copy of US passport information page and signature page- must be at least 6 mo.
- One passport photo (2"x2")- must be taken within last 2-3 months

- One IVS Order Form (click here to download)
- Copy of round trip itinerary
- Yellow Fever Vaccination card (copies accepted)
- Copy of Green Card for non-US citizens
