damaged U.S. Passport for adults & minors

Please note, the damaged passport will not be returned to you.

Use this procedure if:

• You had a previous passport
  
• Your passport's binding is coming apart
  
• The information page of your passport is damaged (photo is not clearly visible, barcode is damaged)
  
• Your passport was washed with laundry, chewed by a pet, used as a coloring book by a toddler, etc.​
  

STEP 1. Gather the following documents:

• Proof of U. S. Citizenship (certified birth certificate with both parent's names; previous original US passport, or original Certificate of Naturalization) plus 1 copy
  
• Two passport size photos 2"X 2"  (must be taken within last 2-3 months), cannot wear glasses (new Passport Agency rule as of 11/01/2016); photos can be done in our office (see fees below)
  
• One application DS-11 (fill out online or download paper version). This form can only be signed in front of the clerk at the post office
  
• Statement from the applicant or parent (in case of minors) as to how the passport was damaged
• Two Letters of authorization - (click here to download) once signed, we have to submit it to the passport agency within 5 business days. For Minors: each parent has to sign a letter of authorization and provide 2 copies. Signature(s) on the letters of authorization and all other documents must match signatures on your ID.
  
• Copy of ID (front & back). For minors: copies of both parents' ID's.
  
• Travel Itinerary showing international travel within 14 days. Questions about itinerary, please call 212-759-5540.
  

STEP 2. Document Pre-Check (required):

Passport Plus Visas Document Pre-Check Service can save valuable time and reduce stress. Use this service to avoid mistakes or delays. Our experienced passport specialists will review and check your scanned documents to confirm that your application is ready to be filed, and will alert you if there is information that needs to be corrected or if there any missing paperwork. E-mail copies of the documents from Step 1 to info@passportplusvisas.com for confirmation. There is no charge for document pre-check for this procedure.
 
STEP 3. Find the nearest post office that can assist you by clicking here.
 

• For Minors: both parents and the child must be present. If only one parent is present, the second parent must fill out and notarize form DS-3053 and add a copy of the ID (front & back) that was presented to the notary.
  
• Show clerk driver's license or valid passport and tell him/her that you want to do a "hand-carry" procedure
  
• Sign the application form DS-11 in front of the clerk, so they can witness your signature
• Make a money order or check for $170.00 for adults or $140.00 for minors made out to "US Department of State"
  
• Present all documents from the list in STEP 1 to the post office clerk
  
• Post office clerk will put all documents into one envelope, seal it, put official post office stamps on it, and give it back to you
  
• Make sure to ask the post office clerk to address the envelope to "Passport Agency" and put your applicant's name and date of birth on it
  
• The post office will charge you $35.00 for this process
  
• Make sure that ALL paperwork including itinerary is included inside sealed envelope. Place one Letter of Authorization inside and one outside the sealed envelope. 
  
• WE MUST RECEIVE YOUR SEALED ENVELOPE WITHIN 24 HOURS AFTER IT'S SEALED. If it takes longer for the sealed envelope to reach our office, the Passport Agency might ask to re-seal the envelope.
  

STEP 4: Fill out Order Form (Download here) and place it on top of your sealed envelope.

STEP 5: Send or deliver all documents for processing to the address below:
