U.S. Passport renewal for  adults- option 1

Use this procedure if:

• Your most recent passport was valid for 10 years
  
• You are in possession of original expired or expiring passport
  
• Your passport expires anytime in the next 6 months - 4 years
  
• You don't have any blank visa pages left
  
• Your U.S. Passport expired less than 5 years ago
• Your name has not changed OR your name changed AND your passport was issued more than 1 year ago
  

STEP 1. GATHER THE FOLLOWING DOCUMENTS:

• Original expired/expiring passport issued within the last 15 years
  
• Two passport size photos 2"X 2" (must be taken within last 2-3 months), no glasses
  
• One Application DS-82 (fill out online)
  
• Two Letters of Authorization (click here to download)
  
• PPV Order Form (click here to download)
  
• ​If your name changed: a certified copy of the marriage certificate, divorce decree or a court order (this document will be returned to you upon passport completion)
